Abstract

Re-audit is necessary to ensure that implemented changes are sustained. n An initial audit determined the accuracy of pneumonia documented as the primary cause of death. A re-audit explored whether the interventions implemented after the first audit were effective, thereby completing the audit cycle. Interventions applied after the original audit were not effective in improving accuracy. Core clinical features were not present in a large proportion of patients who had evidence of pneumonia.
